**Job Summary** Does it bring you joy to help seniors navigate the complexity of the healthcare system? This position is PRN field work as a Care Manager, assisting seniors with coordinating their care between multiple providers and agencies. Enable senior citizens to live comfortably at home and bring peace of mind to family members knowing their loved ones are cared for. Serve as the expert to assist client with obtaining providers to meet their needs. Visit clients in their homes to conduct initial assessments and re\-assessments, write client care plans, recommend and coordinate changes in the level of care, assist clients with understanding available care options and connecting them with providers. Update care plan and supervisory notes in electronic records. New clients may at times require short notice assessments within the company service area of Fairfax County, Arlington, and Alexandria.
